Depending upon each health plan and benefit coverage, co-pays typically range from as little as $5.00 to as much as $35.00.

Since Dr. Hirsa is an approved network provider for most insurances, a PCP referral prior to initial treatment is typically not
required. Please contact our clinic for PCP referral verification. Our staff will be happy to assist you.
Prepaid Cash Plan
We also offer cash pre-paid plan for un-insured and under-insured patients. We accept personal checks, VISA or
MASTERCARD.
